How it works
Tell us who needs help, where care is needed in Oregon, your timing, and the best way to reach you.
The care team reviews your inquiry and follows up to learn more. You can optionally complete the full assessment to provide more details.
We help you understand provider types and possible next steps. Providers confirm their own services, pricing, availability, licensing, and admissions requirements.
Who can request help
Individuals can request guidance for themselves, loved ones can request help for a family member or friend, and discharge planners or referral professionals can use the referral partner portal for appropriate client referrals.
